Reminder – Health insurance plans may cover cost of COVID-19 at-home tests
Cases of respiratory illnesses – including COVID-19, influenza, and RSV – are rising. As we approach Univeristy closure, we want to remind benefits-eligible employees that our Arizona Department of Administration health insurance plans may cover the cost of at-home COVID-19 antigen tests.
Every covered person, including dependents, enrolled in ADOA health insurance qualify for eight free or reduced-cost at-home tests each month.
Two ways to receive your tests:
- Pharmacy: Request your at-home test at any in-network pharmacy.

Reimbursement: Get reimbursed up to $12 for each FDA-authorized COVID-19 test you purchase (online or at a store). You must submit your receipt when requesting reimbursement. Use the following forms to submit your reimbursement request:

You can also submit your request online by logging in to your UnitedHealthcare account

Note: CVS, Walgreens, Publix and Kroger will not process at-home over-the-counter COVID-19 tests through your MedImpact pharmacy benefit. If you purchase tests at these stores or through another pharmacy or retailer that is unable to bill directly under your pharmacy benefit, you will need to pay for the tests and submit a reimbursement request.
