Applications Now Open for Childcare Choice – Employee Program

Benefits-eligible UA employees may now submit their application to the UA's Childcare Choice – Employee program. Visit: lifework.arizona.edu/cc/childcare-choice-employee to begin the process.
The Childcare Choice – Employee program helps UA employees defray costs of work-related childcare occurring in Arizona. Eligible applicants can receive a reimbursement of up to $2,000 annually per household for qualifying childcare in Arizona.
To apply:
- Review the 2019–2020 Program Guidelines for eligibility qualifications, submittal deadlines, and more.
- Complete a 2019–2020 Enrollment Verification Form and have your eligible childcare provider sign it.
- Upload the signed Enrollment Verification Form to your 2019-2020 UA Childcare Choice E-Application.
After you apply:
You will receive a message through your UA email address regarding your application status. Once approved, it will be your responsibility to submit cost forms in accordance with program deadlines. Please refer to the "Reimbursement" header on page 3 in the Program Guidelines for instructions on how to submit cost forms.
